Who you're working with
Bentley & Green Health AI, LLC is an employee benefits advisory and sales firm. Certain members of our team are licensed insurance producers. We help employers evaluate and adopt the Spousal HRA and the Health AI app, and we connect them with the partners who implement and support both.
We aren’t the HRA administrator, an insurance company, a law firm, a tax advisor, or a medical provider. A specialist third-party administrator implements and runs the Spousal HRA: it prepares the plan document and summary plan description, issues HRA cards, processes reimbursements, and handles member service and reporting. The administrator’s name, credentials, and plan documents come with your estimated savings proposal.
The Spousal HRA is not insurance
The Spousal Incentive HRA is an employer-funded Health Reimbursement Arrangement under Internal Revenue Code Section 105, documented in a formal plan document. The employer sponsors and funds it. It is not insurance, and it doesn’t replace the employer’s medical plan.
- Voluntary. Every eligible employee on the plan is offered the same option. Employees who don’t qualify, or don’t choose it, keep the same plan, carrier, and contributions.
- Not a carve-out or surcharge. It adds an option; it doesn’t push spouses off the plan or charge them more.
- What it reimburses. 100% of eligible out-of-pocket costs (copays, deductibles, coinsurance, and eligible prescription cost-sharing) under the other group plan, subject to plan rules.
- Ceiling. Reimbursement is capped by plan design at the ACA out-of-pocket maximum: $12,000 self-only and $24,000 family for 2027 plan years.
- Coverage decisions. The HRA follows the other plan’s coverage decisions, so it can’t make an excluded service eligible. Employees confirm their doctors and prescriptions are in that plan’s network before enrolling.
- Changes. If the other plan ends, the employee notifies the employer within the qualifying event window and can return to the employer’s plan under its rules. Dental and vision can stay on the employer’s plans where plan documents allow.
Who qualifies
Qualifying group plans: A spouse's or domestic partner's employer plan; A parent's group plan, for employees under 26; A second employer's group plan; A qualifying retiree group plan from a former employer. Not qualifying: Medicare as primary; Medicaid; TRICARE retiree-only; VA health care; Individual marketplace or non-marketplace plans; Limited-benefit or short-term plans.
Savings figures are historical
- Approximately $6,000 to $8,000 a year per participating employee, based on historical client data. Results vary.
- Historically 25% to 65% or more of each participating employee's plan cost.
- Approximately 30% of employees typically have access to a qualifying group plan.
- “Cut healthcare costs by 20% or more” is based on historical client results. Results vary by workforce, participation, and plan design.
- Typically 90 to 120 days from decision to launch.
Track-record figures (more than 25 years of experience, 97% client retention, more than $400 million in health claims administered, and employers from 300 to more than 150,000 employees) describe our implementation partner, the HRA administrator, not Bentley & Green.
Past results don’t guarantee future results. Your results depend on your workforce, participation, and plan design.
Illustrations are not projections
The estimator and the savings calculator assume 10% of your employees enroll, at $8,000 per enrolled employee a year. At 10,000 employees or more, they use $7,000 and count 80% of employees as on the plan, to be conservative. On the savings calculator, you can also choose a different savings level from $6,000 to $8,000. These are illustrations, not projections for your organization. Your estimated savings proposal is built from your own non-identifiable census.

The Health AI app
The Health AI app is provided by Smart Vision Health and customized for each employer client. Employee data in the app is collected, anonymized, and reported only in aggregate. The app provides general health information. It isn’t medical advice, diagnosis, or treatment, and employees should talk with their own health care providers about medical decisions.
Testimonials
Testimonials reflect individual clients’ experiences and are credited by role. They aren’t a guarantee that other employers will have the same results.
No legal, tax, or medical advice
Nothing on this site is legal, tax, or medical advice. The Spousal HRA is structured for compliance, and its plan documents are reviewed with the employer’s counsel and broker. As plan sponsor, the employer remains responsible for its plans, with support from the administrator and its own advisors.
